The appearance of a purple toenail is most commonly caused by an injury to the toe. The blood shows up under the toenail causing it to look purple. When an large item is dropped on the toe or the toe gets repeatedly banged against something, it can cause the toenail to turn purple. Another cause can be wearing shoes too tight. It forces the toe to hit up against the shoe and can cause the toe to start bleeding underneath the toenail.
